This is our preferred method of applying for a Puppy. Please review the Puppy Contract, before filling out the Puppy Questionnaire. Once the Questionnaire is completed, please email the PDF to ValleyDayleDoodles@gmail.com.
*Before sending the completed Puppy Questionnaire, please select "Print" and then under printer source select "Save As PDF". Then send that new file to the email above.
